The device was not returned for analysis.Production record and corrective and preventative actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a query of the complaint handling database for the reported lot revealed there is no indication of a lot specific issue.Based on the information provided, a definitive cause for the reported balloon rupture could not be determined; however, the reported separation, surgical intervention, hospitalization, and delay to treatment/therapy appear to be related to circumstances of the procedure.Balloon material ruptures can be affected by numerous factors including, but not limited to, balloon damage during processing of the balloon material, materials, inflation technique, and interactions with other devices or lesion calcification. In this case, a conclusive cause for the reported balloon rupture could not be determined since the device was not returned for analysis and limited information was provided. The balloon rupture likely did not allow the balloon to refold properly resulting in the material interacting with the patient anatomy and/or accessory device and subsequently lead to the reported separation during manipulation.Additionally, a conclusive cause for the reported patient effect(s) of unspecified heart problem and death and the relationship to the product, if any, cannot be determined.A medical review was conducted by an abbott vascular clinical specialist (referenced in (b)(6)).This was a case to treat a patient who presented with left anterior descending (lad) disease.Additional patient information, health history, and procedure images were requested but not provided. The case was intended to treat lad disease.A 3.0x30mm trek balloon dilatation catheter (bdc) reportedly ruptures intra-procedurally which necessitated emergent surgical intervention. The surgery was completed successfully, and the patient transferred to intensive care unit (icu) for post-procedure care. However, the patient expired during the hospital stay.Based on the information provided, the cause of death cannot be attributed to the use of the trek bdc. While the device malfunction contributed to the need for emergency surgery, there is insufficient information to establish a direct or indirect casual relationship between the device and fatal outcome. However, it is reasonable to state that the trek bdc may have contributed to the patient¿s adverse event which led to emergent surgery.Further determination of causation would require additional clinical information, including postoperative care details and cause-of-death reporting. The reported patient effects of unspecified heart problem and death are listed in the instruction for use (ifu), trek/mini trek rx, cf/fda/mdr as known patient effects. There is no indication of a product quality issue with respects to the design, manufacture, or labeling of the device.
