Event or Problem Description
It was reported that during a procedure, the bag tore and subsequently came off.The bag was thrown away and was noted to be full of blood.

Event or Problem Description
It was reported that during a laparoscopic cholecystectomy, the bag tore and subsequently came off once the user closed up the string.The bag was thrown away and was noted to be full of blood.A new bag was used to resolve the issue.There was no patient injury.
