Event or Problem Description
It was reported that defibrillation threshold (dft) testing was performed due to the patient experiencing unsuccessful high voltage therapy and the right ventricular (rv) lead was reprogrammed with the superior vena cava (svc) coil turned off.On the same day, a rv lead warning triggered for high rv defibrillation impedance.During additional successful dft testing, the rv lead was reprogrammed with the svc coil programmed on and high voltage impedance normalized.Fluoroscopy revealed that the cardiac resynchronization therapy defibrillator (crt-d) had migrated from its original position. The rv lead and crt-d remain in use.No patient complications have been reported as a result of this event.

Event or Problem Description
It was further reported that the svc coil was turned back on.
