Block h3: the bub (bedside utility box) was returned for evaluation without the ivus v3 connector installed in the device.Visual inspection found no unusual characteristics of the device.During functional testing, the bub was connected to the intrasight console, the led on the fm-pim lit up in blue, indicating that the device was recognized successfully. However, further testing of the ivus mode could not be performed due to the missing ivus v3 connector.Block h6: based on the available testing, no problem was detected. However, due to the incomplete testing, the probable cause of the reported failure "ivus not available error" could not be established.Submission of this report does not, in itself, represent a conclusion by the manufacturer and/or authorized representative or the national competent authority that the content of this report is complete or accurate, that the medical device(s) listed failed in any manner and/or that the medical device(s) caused or contributed to an alleged death or deterioration in the state of the health of any person.

Block h3: the intrasight workstation was returned for evaluation.Visual inspection found no unusual characteristics of the device.During functional testing, the workstation was connected to the lab system, and immediately turned on without pressing the main power button; however, no video signal was present on the display monitor, even after multiple power up attempts, and disconnecting/reconnecting video output cables.Block h6: based on the available testing, the probable cause of the reported failure "ivus not available error" could not be conclusively determined since the device displayed no video signal to continue testing. However, the probable cause of the no video signal is likely due to an internal component failure.Submission of this report does not, in itself, represent a conclusion by the manufacturer and/or authorized representative or the national competent authority that the content of this report is complete or accurate, that the medical device(s) listed failed in any manner and/or that the medical device(s) caused or contributed to an alleged death or deterioration in the state of the health of any person.
