Additional Manufacturer Narrative
Additional information d9, h3, h6, and h11.H11: the device was received for evaluation.A visual inspection with the naked eye was performed which observed a broken occluder leg which caused a leak through the set.Functional tests were performed which included leak, clear passage, and clamp function tests and a leak through the closed clamp was observed. The reported condition was verified. The cause of the leak was due to the broken occluder leg.The cause of the broken occluder leg could not be determined, however, if the set was exposed to chemical agents such as hydrogen peroxide, alcohol or bleach, as listed on product packaging, this could damage the transfer set materials.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
