It was reported that, during a knee arthroscopy acl, was prepared, passed through the tunnels, and first fixed in the femoral tunnel. Then, the graft was fixed in the tibial tunnel.A 9mm tap was used beforehand, and an 11x30mm biosure screw was inserted. As the biosure screw was being inserted, it was noticed spiral-shaped fragments detaching, which initially thought were suture material. However, these were actually fragments of the screw itself. The screw was fully inserted and appeared to be secure, but upon intra-articular inspection, the graft was found to be completely loose. Therefore, that screw had to be removed. All the pieces were removed from the patient with a kelly clamp. The procedure was resumed, with a non-significant delay, using a similar sn back-up.The back up device was used in the originally drilled bone hole. The instrument to prepare the insertion site was a cannulated tibia drill bit.The patient was not exposed to additional anesthesia. The patient is stable. No further complications were reported.

H11 h3, h6 the reported device was received for evaluation.A visual evaluation showed the device was returned in original packaging with the batch number of the complaint on the label.A thread has separated and fractured. The fractured material was returned.Based on the condition of the product material found during visual inspection, additional material testing is not required.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A review of the material specifications found that the storage requirements for the material are specified, and the material must comply with provided percentage composition specifications as measured by ash test.A definitive root cause for the reported issue could not be determined.Factors that could have contributed to the failure include procedural variance, or deviations from established protocols, increasing risks and leading to unintended consequences (excessive force on the device, excessive torque on the device, attempted correction of a damaged device, off-axis insertion, improper preparation of the insertion site, or an inadvertent impact event inconsistent with normal use).Based on this investigation, the need for corrective action is not indicated.
