It was reported that an eagle eye platinum catheter was used in a coronary therapeutic procedure.During use, the catheter would not back off the non-philips guidewire; therefore, removed together. The vessel was rewired and a new catheter was used to complete the procedure. No patient injury reported. This product problem is being submitted because the eagle eye catheter and guidewire were removed as a system. There is potential for harm if it were to recur.
