(b)(4).Date sent: 3/5/2026.This is an analysis of two photos submitted to ethicon endo surgery for evaluation.During the visual analysis, the following was observed: the photo shows a staple line on the tissue and the tip of staple legs could be observed along staple line.Based on the photo the event described is confirmed, however no conclusion or root cause could be determined.Hands on device analysis may provide the additional evidence necessary to confirm the root cause of the reported event.Because the instrument was not returned our evaluation is limited. As part of ethicon endo surgery quality process all devices are manufactured, inspected, and released to approved specifications.Device history review: an evaluation of the manufacturing record could not be performed as the required product identification number was not provided to complete the evaluation.Additional information was requested and the following was obtained: another the internal rrt call was held with the ethicon team.As an update on the patient, she did mention that the patient ended up with more puss/bacteria post op and is in icu.She is recovering and healing.Dr.Does not know for certain the cause.Today dr.Stated that 2 gcl100s were used during this case. She used one that did not fire properly so she asked for the second one.The second one did not form properly (picture that was provided) so she asked for the glc80.It is uncertain, though believed, that the stapler returned was the 2nd glc100 used.Event description states 2 glc100s were used ¿ did both experience the same issue? yes. There is a glc80 device mentioned in the event description. Were there any issues with the glc80? no issues were experienced.Dr.Does not prefer to use the 80 b/c you have to use more reloads and cross stapler lines which leads to the potential of more leaks (she does oversew her staple lines).Any recollection if staple form looked the same on the excised tissue portion of the stomach ¿ ex: did the sample form look the same as the photo with open legs/non-b form, or did it look like properly closed staple shape? ¿ dr.Says, yes, specimen side also showed malformed staples. No pix provided. Were there any comments from the surgical team regarding the packaging ¿ for example, loose parts seen within the package?- nothing known.
