Event or Problem Description
It was reported that on (b)(6) 2026, a 35mm amplatzer cribriform was chosen for implant using an 9f amplatzer trevisio delivery system.During the procedure, while deploying, it was noted that the occluder did not conform to its desired shape but it deformed like "invert martini glass".The device was recaptured trice and had to retrieve outside of the patient's body to examine. The occluder was massaged in a saline bath to help get back to its desired shape and then it was re prepared. When attempted again, the same issue was observed. The procedure was completed using a larger device. The deformed device was never released from the delivery cable. There was no interaction with the cardiac structures during deployment and no angulation/kink were noticed in the delivery system. The patient remained hemodynamically stable throughout the procedure. There was no clinically significant delay in procedure and no adverse patient effects.
