Event or Problem Description
It was reported that a patient underwent a cardiac ablation procedure with a thermocool smarttouch sf (stsf) catheter and the patient experienced cardiac tamponade which required prolonged hospitalization. The report indicated that during ablation with a thermocool smarttouch sf catheter, a steam pop was noted during ablation with 50w.After a few minutes, the systole dropped to 50, and echo confirmed a pericardial effusion (pe). The physician stopped the procedure.Fluid was drained and the patient was then taken into intensive care and was stable before and upon transfer, systolic pressure was fine as well.Additional information received indicated that the physician¿s opinion on the cause of this adverse event was too much force and ¿heat from his side¿.The patient fully recovered (no residual effects). The sheath and the catheters exchange were approximated 6-7 times. No transseptal puncture site was performed during the procedure. Two ablations were performed during the procedure with radiofrequency (rf) energy in the left ventricular outflow tract (lvot), outside the pulmonary veins. There were no ablations performed within the pulmonary veins. The force sensing ablation catheter used was stsf. The force visualization features used were dashboard, vector, and visitag. The visitag module parameters for stability used was time, 3s, 3mm, 3 size.No additional filter was used with the visitag. The color option used prospectively was time. The flow setting was 8ml/min.The patient did not require cardiac surgery. The correct catheter settings were selected on the generator. No issues with flow rate change at the start of ablation. No error messages observed on biosense webster equipment during the procedure. The steam pop is not mdr reportable.

Additional Manufacturer Narrative

It was reported that a patient underwent a cardiac ablation procedure with a thermocool smarttouch sf (stsf) catheter and the patient experienced cardiac tamponade which required prolonged hospitalization.Device evaluation details: the product was returned to johnson & johnson medtech (j&j medtech) for evaluation.A visual inspection and revision of all features were performed following j&j medtech procedures.Visual analysis revealed no damage or anomalies on the device. The device features were reviewed, and no issues were observed during the product investigation.A manufacturing record evaluation was performed for the finished device batch number, and no internal actions were identified. The steam pop issue reported by the customer could not be replicated during the product investigation; other issues or circumstances may have occurred during the usage of the device that compromised its performance. No malfunction was observed during the product analysis. The root cause of the adverse event remains unknown. In addition, no device malfunction was reported. There may have been other circumstances or issues that occurred during the use of the device that could not be replicated during the analysis. The instruction for use (ifu) states that careful catheter manipulation must be performed to avoid cardiac damage, perforation, or tamponade; at higher contact force settings, rapid power increases during ablation may result in steam pops, which increase the risk of perforation. As part of johnson & johnson medtech¿s quality process, all devices are manufactured, inspected, and released to approved specifications. If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
