Additional Manufacturer Narrative
Block h6: imdrf device code a040601 captures the reportable event of a wire anchor dislodged.Block h11: investigation results: the truetome jag 44 device was not returned for analysis; therefore, a technical analysis could not be performed.Good faith effort attempts were made to try and retrieve the device; however, response was not received.A media analysis was performed on the video provided by the complainant. The video revealed that the device in patients' anatomy with the wire anchor dislodged. No other problems with the device were noted.According to the media analysis performed, it was found that the wire anchor dislodged, however, there is not enough evidence to determine whether the problem was induced due to the physician's technique during the procedure or was related to a device malfunction. The investigation concluded that, without analysis of the device, the most probable root cause of the clinical observations is cause not established.A review of the device history record (dhr) was performed. The review of the dhr identified that there were no process related non-conformances, scrap, or rework performed during the production that could explain the event. The reviews ensure each device meets specification prior to release for use. There is no indication the device manufacturing process contributed to the reported complaint.
