It was reported that during an unknown procedure, the forceps were accidentally fired while clamped together with the tissue, causing the knife to stop. The knife did not return when using the knife reverse switch, so the manual override was used to return the knife. However, the jaws did not open, so the forceps were fired while clamped, causing the knife to stop. When interviewed, the comment was, "i think the knife reverse switch was pressed afterwards, but the jaws would not open." the cartridge color was gold.Another device was used to complete the case. There were no adverse consequences to the patient. No additional information was provided.

(b)(4).Date sent 3/10/2026.D4 batch #739d19.Investigation summary: the product was returned for evaluation.Visual inspection and functional testing were conducted on the returned device.Visual analysis of the returned sample determined that one pcee60a device was returned with no apparent damage and with a gst60d reload present. The reload was received with partially fired 1/2 and with damage on reload deck.Upon evaluation of the reload, the reload body, one-piece sled, and some drivers were noted to be damaged. The manual override door was out of position; the override lever was up which denotes that the knife was manually returned to home position. It should be noted after the manual override system is used the instrument is disabled and cannot be used for any subsequent firings. The bailout system was reset and then, it was tested for functionality in the straight position with a test reload and achieved its complete firing sequence without any difficulties. The staple line and cut line were complete and the staples meet the staple release criteria. The damage to the reload is consistent with the device being clamped over a hard object. To mitigate the potential for staples getting into the reload and interfering with the knife path during device firing, prior to reloading the device, rinse the anvil and reload jaw in sterile solution and then wipe the anvil and reload jaw to clean any formed but unused staples from the device.Additionally, proper care should be taken when placing the device on the tissue to be stapled, to ensure that no hard obstruction such as a clip is included with the tissue inside the jaws. Please reference the instruction for use for more information. The event described of unintentional activation, difficult to open, would not reverse knife automatic and would not reverse button force, could not be confirmed as the device performed as intended and it opened and closed without any difficulties noted.Once the device completed the firing sequence the knife returned home as intended. The knife reverse button worked properly during testing. As part of ethicon endo surgery¿s quality process, all devices are manufactured, inspected, and released to approved specifications.A manufacturing record evaluation was performed for the finished device batch number 739d19, and no non-conformances were identified.
