Block b3: approximated based on the date the manufacturer became aware of the event.Block h6: imdrf device code a0401 captures the reportable event of cutting wire break.Block h11: investigation results- the ultratome xl device was not returned for analysis; therefore, a technical analysis could not be performed.Good faith effort attempts were made to try and retrieve the device; however, response was not received. However, a media analysis was performed on the photo provided by the complainant. The photo revealed that the cutting wire was broken and kinked. No other problems with the device were noted.According to the media analysis performed, it was observed that the cutting wire was broken and kinked, however, there is not enough evidence to determine whether the issues were induced due to the physician's technique during the procedure or were related to a device malfunction. The investigation concluded that, without analysis of the device, the most probable root cause of the clinical observations is cause not established.A review of the device history record (dhr) was performed. The review of the dhr identified that there were no process related non-conformances, scrap, or rework performed during the production that could explain the event. The reviews ensure each device meets specification prior to release for use. There is no indication the device manufacturing process contributed to the reported complaint.
