Block d4: detailed product information was not provided to bsc.We completed a good faith effort to obtain the information.Because the product is unknown at this time, we are unable to provide the complete unique identifier (udi) # and other product specific information. If additional details become available, a supplemental report will be submitted. The device has not yet been evaluated.Upon completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental medwatch will be filed.

Event or Problem Description
It was reported that the coating on the catheter shaft was twisted.Consequently, when the catheter was inserted into the agilis introducer sheath, the twisted coating appeared to cause a blockage, and flushing was unable to be performed.During a pulsed field ablation (pfa) procedure to treat atrial fibrillation, an intellamap orion catheter was used.It was noted that the coating on the catheter shaft was twisted.Consequently, when the catheter was inserted into the agilis introducer sheath, the twisted coating appeared to cause a blockage, and flushing was unable to be performed.Another of the same device was used, and the procedure was completed successfully with no patient complications. The device has been received, pending analysis.
