Event or Problem Description
It was reported that after the chest was closed, the arterial pressure dropped significantly due to vasoplegia. The heartmate 3 increased flow and the right ventricle enlarged which caused a lot of harm. The patient more or less recovered with a lot of drugs.A right ventricular assist device (rvad) with extracorporeal membrane oxygenation (ecmo) was placed with protek duo due to right heart failure. The rvad was a centrimag pump. The lungs of the patient were damaged, and they did not saturate well. The patient had right heart failure, bleeding at the outflow graft of the heartmate 3, hypovolemic shock, cardiac collapse, and vasoplegia. The patient was treated by reopening the chest, echocardiogram, and monitoring hemodynamic parameters. The patient passed away. The outcome was not considered device or therapy related and the device operated as expected. An autopsy was not performed, and the device was not explanted.

Additional Manufacturer Narrative
Manufacturer's investigation conclusion: a specific cause for the reported events could not be conclusively determined through this evaluation, and a direct correlation with the centrimag blood pump could not be conclusively established.Prior to centrimag right ventricular assist device (rvad) support, the patient was implanted with a left ventricular assist device (lvad) and reportedly experienced significant arterial pressure decrease due to vasoplegia, an enlarged right ventricle/right heart failure, and damaged lungs with poor saturation. These events led to the patient being placed on a centrimag rvad with extracorporeal membrane oxygenation. The patient then reportedly experienced bleeding at the lvad outflow graft and hypovolemic shock. An echocardiogram was reportedly performed, and the patient¿s chest was reopened. The patient ultimately expired due to consistent vasoplegia and cardiac collapse. It was reported that the patient¿s death was not considered to be device related, and it was reported that the device operated as expected. The centrimag pump was not returned for evaluation. The lot number or other identifying information of the product was not reported and was not able to be determined during the investigation. The centrimag blood pump instructions for use (ifu) is currently available. The ifu lists bleeding and death as adverse events that may be associated with the centrimag circulatory support system under ¿adverse events.¿ the ifu also provides the following warnings and cautions: ifu warning #10: it is intended that systemic anticoagulation be utilized while this device is in use.Anticoagulation levels should be determined by the physician based on risks and benefits to the patient.Ifu warning #12: frequent patient and device monitoring is recommended.Do not leave the device unattended during use.Ifu caution #5: this device should only be used by persons thoroughly trained in extracorporeal circulation procedures. The current revisions of the ifu can also be found on the electronic ifu (eifu) page of the abbott website. No further information was provided. The manufacturer is closing the file on this event.
