Additional Manufacturer Narrative
The reported event was confirmed during the investigation. The device inspection revealed the following: the identification of the returned screw was confirmed based on the catalog number and catalog number marked. The returned bone screw is fractured below the head, which was the only part returned. The lines of rest visible on the breakage surface indicate a fatigue fracture caused by continuous excessive loading, as confirmed by the presence of rest lines. It should be noted that more detailed information about the event, as well as x-ray images, would have been necessary to conduct a complete investigation. The exact root cause of the incident could not be determined.During the device inspection, it was not possible to confirm any user-related error. However, this cannot be ruled out.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required currently.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design-related problems were found during the investigation. If more information is provided, the case will be reassessed.
