H3.Device evaluation summary: medtronic conducted an investigation based on all information received. It was reported that the 980 ventilator would not stay powered on while operating on battery power and would shut off as soon as it was unplugged from the wall outlet.A review of the ventilator logs indicates that there was a loss of ventilation (lov) while in use.The device was available for evaluation.Service personnel (sp) inspected the unit logs and found background codes related to the p rimary battery, as well as several ¿system starting up after power failure¿ entries.During the battery test in extended self test (est), when disconnected from the ac power, sp found the ventilator shut down, thus confirming the reported issue.Additionally, it was then noted that the batteries were not charging while connected to ac power.Based upon these findings, sp replaced the breath delivery (bd) power controller printed circuit board assembly (pcba) and the bd power distribution pcba.Advised the customer to replace the battery as a precaution. The ventilator passed all calibrations and tests per manufacturer specifications at the time of service. The cause of the reported event and the lov was isolated in the field to a potential fault in bd power controller (pcba) and/or bd power distribution pcba. The event is included in a data monitoring plan.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
