H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of a break in the catheter is unconfirmed because the problem could not be found or reproduced. One 4 fr d/l powerpicc provena was returned for evaluation. An initial visual observation of the returned catheter showed blood residues throughout the device. The catheter appeared cut at a sharp angle at the 30 cm depth marker.A functional test of pressurizing the catheter with water using a 12 ml syringe revealed no leaking throughout the catheter.A microscopic observation revealed the cut of the catheter at the 30 cm depth marker was performed using a sharp, bladed instrument as striations were observed at the cut.The cut was observed to be intentional.Because no damage was observed along the returned catheter, the complaint of a catheter break is unconfirmed. This complaint will be recorded for future trending and monitoring purposes.
