Block e1: the initial reporter facility name is (b)(6).Block h6: imdrf device code a040601 captures the reportable investigation results of cutting wire kinked.Block h11: investigation results the returned jagtome rx 44 was analyzed, and a visual inspection found the cutting wire was kinked. The device was inspected under magnification and was observed that the proximal pierced hole (tomes extrusion) was torn, this condition displaced the cutting wire from its original position during actuation. The combination between the kink in the cutting wire and the torn section in the working length resulted in the wire inability to bow.No other problems with the device were noted. The product analysis of the returned device revealed the cutting wire kinked. This condition could be caused by operational factors, the used technique for the device manipulation or by the interaction between the device and a hard surface. The proximal pierced hole was torn (working length torn), due this condition the wire was displaced from its original position. This problem could have been caused by submitting the catheter to tension forces during the handle actuation, also bowing the device without being completely out of the scope. The combined condition limits the overall performance of the device making the device unable to bow.Based on all gathered information, the most probable root cause is adverse event related to procedure.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.A risk review was completed and confirmed that the event of wire kinked was defined in the risk documentation. This event type has been accounted for during product risk analysis to support acceptable risk benefit for the product.
