Additional Manufacturer Narrative
Updated fields: d4, d9, g3, g6, h2, h3, h4, h11.The disposable perforator (id (b)(6)) was returned for evaluation.Device history review: no anomalies were identified during the manufacture or packaging of the device.Failure analysis: abnormal levels of heat were not observed.Slowing drill speed or irrigating the perforator can reduce heat buildup. The complaint cannot be confirmed.Root cause analysis: returned product testing identified no nonconformities and no abnormal heat generation. The cranial perforator is designed to interface with bone only and is not intended to contact patient skin when used in accordance with the instructions for use. While the issue was not reproduced potential causes of the excess heat is due to friction.Irrigation or reducing drill speed can decrease the amount of heat built up in the drill.Under intended use conditions, there is no identified mechanism for patient skin exposure to heat.
