It was reported that a clearlink system - non-dehp solution set leaked during secondary infusion of taxol.The location of the leak was described as "where the two ports connect on the primary tubing".The device was replaced to resolve the issue.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
