It was reported that admin staff opened two needles at the main station resulting in failures.I had them describe to me the technique used for insertion, and they did state they pulled straight up on all.The needles are twisted inside of the shaft and will not come apart, except when using pliers to force them apart. This report addresses both devices.

H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of a stuck stylet/obturator is confirmed; however, the exact cause is unknown. Two photographs and one video of an io needle were returned for evaluation. An initial visual observation of the photographs and video showed the needle with a stylet/obturator installed. Some slight use residue was observed on the needle cannula. The obturator appeared slightly over-clocked within the needle cannula, and there appeared to be some damage to the needle tip; however, this could not be clearly discerned from the returned photographs and video. One of the photos and the video showed the stylet/obturator slightly removed from the needle. While the obturator of the device appeared to be positioned in such a way that it would be difficult to remove, there were no distinguishing features in the returned photographs and video of the device which could aid in identification of a specific root cause. This complaint will be recorded for future trending and monitoring purposes.
