Additional Manufacturer Narrative
The device malfunctioned, which resulted in the need for medical intervention in the form of a venous cutdown. The suspect device was returned and evaluated. The root cause of the catheter separation was most likely due to procedural factors such as excessive manipulation during the procedure.Excessive force used to advance or retract the device against resistance can cause damage to the catheter.Per device labelling: "do not advance without dilator fully inserted.Avoid using excessive force to advance or retract against resistance." the device identifiers were provided, and the lot history records were reviewed. There were no discrepancies or unusual findings.Additionally, a complaint history review was conducted and no similar issues have been reported against this lot number.
