Event or Problem Description
It was reported that a continu-flo solution set leaked during an unspecified process step.The line had been primed with 5% dextrose in water prior to connection with the patient.The nurse noticed that the tube was leaking from a slice or hole near distal end of the set.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

Additional information: h11: the device was received for evaluation.Visual inspection was performed on the returned sample and a cut in the tube was observed.Pressure test was performed and leak was noted due to the cut in the tube.The reported condition was verified.The cause of the issue was related to manufacturing.Should additional relevant information become available, a supplemental report will be submitted.
