Event or Problem Description
Medtronic received information that during use of a custom tubing pack, it was reported that the sucker has broken 3 times on recent cases, and all with the same lot number.The tip of the sucker cracked completed across, and it was a complete break.The device was replaced to complete the procedure.There was no adverse patient effect associated with this event.
