H11: manufacturing review: the device history records have been reviewed, and this lot met all release criteria.Investigation summary: the physical device was not returned for evaluation. One photo was provided for review. The photo shows two segments of a peel apart sheath that have already been separated.Since the sheath is in a post peeled condition, no deformation or abnormality can be assessed from the available segments. The investigation is inconclusive for the reported sheath deformation, difficult to insert and failure to advance issues as the provided photo was not sufficient to confirm the reported issues. The definitive root cause could not be determined based upon available information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.D4 (unique identifier (udi) #), g3: h6 (method, result, conclusion)
