Additional Manufacturer Narrative
Correction: the aware date was incorrectly entered in the initial mdr.Device evaluation: the complaint of delayed needle retraction was confirmed, and the cause appeared to be manufacturing related. One video and two fully retracted needles were provided for investigation.Examination and functional testing of the returned samples was performed. No defects or abnormalities were observed. The video showed the activation of the safety mechanism after the iv catheter had been removed from the needle. The retraction time of the needle exceeded the specification, which was likely caused by excess or dispersed gel.A trend has been identified for the reported failure and further actions have been initiated to investigate this type of incident and address the manufacturing root cause. No additional action will be taken at this time.A review of our risk management documentation was performed and indicates that the potential risk of the reported event was assessed appropriately.Complaints received about this device and reported condition will continue to be tracked and trended.Information will be captured on trend reports and monitored. Our business team regularly reviews the data collected for identification of emerging trends.
