unique identifier (udi) #: (b)(4).Upon receipt at our quality assurance laboratory, this inflatable penile prosthesis (ipp) underwent a thorough analysis. The cylinders were microscopically inspected and leak tested.Microscope inspection revealed that a cylinder had a hole in the middle from a sharp instrument.Leakage testing revealed a leak in a cylinder from a sharp instrument in the middle of the cylinder. The pump was microscopically inspected and functionally and leak tested.Microscopic inspection revealed that the pump tubing was cut down to the pump block; the tubing was not returned for analysis. The pump failed the activation tests for the functional test. The pump passed the leak testing. The device history record (dhr) review confirmed that the device met all material, assembly and performance specifications.A review of the device instructions for use (ifu) was completed and did not reveal any evidence of device misuse, off label use, or failure to follow instructions.A risk review was completed and confirmed that the event of mechanical malfunction (leaks, incomplete inflation/ deflation, kinking) was defined in the product risk documentation. This event type has been accounted for during analysis to support acceptable risk benefit for the product.Based on the information available and analysis results, the failure to pass the activation test identified in the pump could have caused or contributed to the reported clinical observation of mechanical malfunction.
