On (b)(6) 2026, the patient underwent an peripheral artery disease procedure to treat a right lower extremity acute limb ischemia and a thrombosed arterial system, utilizing gore® dryseal flex introducer sheath.Allegedly, during the procedure, shortly after insertion and removal of the dilator, physician noticed something appeared abnormal. The physician attempted to advance a 12 fr penumbra thrombectomy catheter through the sheath but was unable to pass it across the aortic bifurcation. An angio shot was performed for better visualization, which revealed that the sheath was kinked. It was also reported the penumbra catheter was removed, and the physician attempted to gently withdraw the sheath to straighten the kink.Only minimal force was applied, however the sheath separated, and only the proximal half was retrieved.A pta balloon was then advanced to facilitate the removal of the remaining sheath fragment. The remainder of the sheath was successfully extracted through the access site without further complication.A new dryseal sheath was then placed, and the case proceeded without additional issues. The patient tolerated the procedure.
