Event or Problem Description
It was reported that a patient underwent a breast augmentation revision surgery with implantation of a 250cc mentor smooth round moderate profile saline breast implant prosthesis.Post-operatively, the patient was diagnosed with left breast baker grade ii capsular contracture and a deflated left implant by a medical professional using the patient's symptoms. As a result, the patient underwent left-sided exchange with a mentor saline implant on (b)(6) 2026.

Additional Manufacturer Narrative
On may 11, 2026, mentor was provided information that a device received by the mentor analysis lab belonged to this complaint. With the returned device, the product identity was determined as the following: lot: 6986026.On may 27, 2026, mentor completed an evaluation on the returned device.Device evaluation: a thorough investigation was conducted.Visual analysis of the returned device revealed that the breast implant had a crease/fold on the anterior view.Leak testing was performed, according to mentor procedure, and it identified a tear at the end of the crease/fold, measuring approximately 0.2 cm.No more leak sites were observed. The evaluation determined that the possible cause of the rupture is consistent with normal wear.Deflation can occur at any time after implantation, but it is more likely to occur the longer the implant is implanted.Creating wrinkles or folds should be avoided during implantation or other procedures as it can result in a deflation in a later time.Breast implants may also simply wear out over time.Regarding the reported capsular contracture condition, there are not enough details to determine the factors that may have caused this condition.Capsular contracture in the patient¿s breast might be the result of the body¿s individual physiological response to the implantation of a foreign object in soft tissue.Further, capsular contracture is a known complication associated with these devices and is referenced in our current product insert data sheet.A manufacturing record evaluation (mre) was performed, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures. As part of mentor's quality process, all devices are manufactured, inspected, and released according to approved specifications.Based on the results of this investigation, it was determined that the product met the manufacturing release criteria, and that normal wear may have caused the observed defect. No corrective and preventive action (capa) is required now.Manufacturer¿s reference number: (b)(4).
