Block h6: imdrf device code a0401 captures the reportable event of cutting wire break.Imdrf device code a040601 captures the reportable event of cutting wire kinked.Block h11: investigation results the truetome 44 device was not returned for analysis; therefore, a technical analysis could not be performed.Good faith effort attempts were made to try and retrieve the device; however, response was not received.A media analysis was performed on the photos provided by the complainant. The photos revealed that the device was observed with the cutting wire broken and kinked. No other problems with the device were noted.According to the media analysis performed, it was found that the cutting wire broken and kinked; however, there is not enough evidence to determine whether the problem was induced due to the physician's technique during the procedure or was related to a device malfunction. The investigation concluded that, without analysis of the device, the most probable root cause of the clinical observations is cause not established.A review of the device history record (dhr) was performed. The review of the dhr identified that there were no process related non-conformances, scrap, or rework performed during the production that could explain the event. The reviews ensure each device meets specification prior to release for use. There is no indication the device manufacturing process contributed to the reported complaint.

Block h2: block b5 and block h6 (device code) have been updated based on additional information received march 31, 2026.Block h6: imdrf device code a0401 captures the reportable event of cutting wire break.Imdrf device code a040601 captures the reportable event of cutting wire kinked.Block h11: investigation results the truetome 44 device was not returned for analysis; therefore, a technical analysis could not be performed.A media analysis was performed on the photos provided by the complainant. The photos revealed that the device was observed with the cutting wire broken and kinked. No other problems with the device were noted.Based on the event description and information provided by the customer, the device was used in a manner inconsistent with a written instruction in the ifu.Ifu states: "precaution: it is recommended to maintain direct and constant contact with tissue when applying electrocautery current.Failure to do so may result in broken cut wire, damage to the endoscope and/or patient injury." however, the complainant reported that the device was energized when not in contact with tissue, after media analysis it was found that the cutting wire was blackened and broken.Since the instructions for use were not followed, this could have impacted the device's overall performance and contributed to the reported complaint event, energizing the device when not in contact with tissue can compromise the cutting wire's integrity, also it can may result in broken cut wire, damage to the endoscope and/or patient injury, leading to break it.Additionally.It was observed that the cutting wire was kinked, once the cutting wire breaks, any attempt to remove the device from the scope can kink the cutting wire.Based on all gathered information and the analysis performed, the most probable cause of this complaint is failure to follow instructions.A review of the device history record (dhr) was performed. The review of the dhr identified that there were no process related non-conformances, scrap, or rework performed during the production that could explain the event. The reviews ensure each device meets specification prior to release for use. There is no indication the device manufacturing process contributed to the reported complaint.Review of the instructions for use (ifu) confirmed there was relevant content and sufficient guidance with respect to the circumstances described within this complaint. No updates are required to the ifu as a result of this event.
