Product event summary: clinical data was reviewed.Analysis of the device memory indicated that the ventricular tachyarrhythmia therapy energy was low due to an unintended current pathway. The ventricular tachyarrhythmia therapy energy was also high, and the high voltage capacitor charge time was longer than expected for a device not yet at elective replacement indicator (eri.) a medical safety assessment was performed for this event. In summary, ventricular tachycardia (vt) occurred and was treated with anti-tachycardia pacing and one high voltage shock, where the delivered energy was higher than the programmed energy and there was a tilt reset. The shock was unsuccessful at terminating the vt and the device was unable to deliver subsequent high voltage therapy due to the triggering of charge time-out.The arrhythmia continued for 37 minutes, degrading into fine vf, and electrograms show external defibrillation converting the arrhythmia to a paced rhythm, then the arrhythmia returning despite several external defibrillation attempts, then no cardiac activity. The device behavior cannot be disassociated from the patient death. It was determined that the cause of the over-delivery of high voltage energy with the tilt reset is related to device design; a non-typical reset of the integrated circuit occurred.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
