(b)(4) follow up mdr for device evaluation: it was reported that leakage occurred at the tubing connection site involving the c35-o optima connector and the n40-o optima locking injector.Five photographs were provided to the quality team for investigation.Review of the photographs showed that both the c35-o and n40-o were assembled to a mating device, and no issues with the connection could be identified.Gauze was observed wrapped around the connection between the n40-o and the mating tubing, with a white residue observed near the connection area, suggesting a leak may have occurred. There is no visible damage or other defect identified to indicate why there may have been a leakage.A device history record review was conducted for injector lot 2503736 and connector lot 2506503.No deviations or nonconformances were identified during the manufacturing process that could have contributed to the reported concern.Products undergo multiple inspections throughout the manufacturing process to ensure quality and functionality, including verification of all critical dimensions such as luer threading. Results for the reported lots were reviewed, and no issues related to the event were found.Retained samples of each lot were also used for evaluation. All product was inspected, no damage or defects were observed on the luer threading and dimensional testing verified the product met required specification.Based on our investigation along with the photo evaluation, review of device records, and analysis of the retained samples we were unable to identify a root cause related to the product or manufacturing process at this time.Complaints received for this device and reported condition will be monitored by our quality team for signs of emerging trends.
