Event or Problem Description
It was reported that, during preparation, the tracheostomy tube's stylet was not sealed during the insertion and the guidewire exited from the base of the stylet instead of through the designated openings.It was further reported that it was not possible to maintain the stylet seal despite insertion into the cannula during preparation of the procedure.There was no patient involved.
