The device was returned to olympus for inspection.Based on the results of the investigation, a definitive root cause could not be determined.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.

It was observed that during the device evaluation, the gastrointestinal videoscope exhibited scattered debris, tear marks, and metal foreign material in the forceps passage.There was no patient involvement.
