given the nature of the reported incident, the device could not be returned for evaluation; therefore, a device analysis could not be performed. However, according to the information provided an inappropriate implant size was used during the procedure, which necessitated the planning of a revision surgery.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of complaint history for the part number over the past 12 months and for the batch number based on historical data of the device did not reveal similar events for the listed device.A review of the instructions for use documents for journey ii bi-cruciate stabilized (bcs) total knee system revealed in warnings and precautions that the surgeon should be familiar with the appropriate surgical techniques prior to use of this device and an adequate inventory of implant sizes should be available at the time of surgery.Additionally, revealed that the correct selection of the implant is extremely important.Failure to use the optimum size component may result in loosening, bending, cracking, or fracture of the component and/or bone.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event.Device mishandling or misuse such as using the improper/incorrect procedure ( implanting mismatched sized inserts and baseplates) are documented within the complaint management system to facilitate failure monitoring and trending.Based on the nature of the failure reported and the medium risk of the complaint, no further investigation is required nor updates to instruction for use and surgical technique. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include surgical technique used or user/procedural variance.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary.
