Additional Manufacturer Narrative
|
|
The permanent cautery hook has been returned and evaluated by the failure analysis team. The instrument passed energy delivery testing in various grip orientations. The instrument also passed the electrical continuity test.Based on the investigation results, customer reported issues may be due to other factors unrelated to the product.Additional observation(s) not reported by site: the instrument was found to have thermal damage to various components. The instrument was found to have thermal damage on the monopolar yaw pulley at the cable routing. The conductor wire insulation was found to have thermal damage near the monopolar yaw pulley. The instrument was found to have thermal damage at the distal idler pulley. The instrument was found to have thermal damage on the distal clevis near the idler pulley pin.The instrument was found to have thermal damage on the proximal clevis surface. The probable root cause of the thermal damage is primarily attributed to the use conditions.Clinical use of monopolar energy can also result in thermal damage if the lowest power or effect setting possible is not used for the minimum time necessary to achieve the desired effect.Conductor wire management issues can result in damage to the conductor wire, which may allow a possible arc path during air firing.
