Event or Problem Description
It was reported that during a total hip replacement surgical procedure it was observed that the strike plate was broken off broach handle, the reamer handle would not fit in the chuck, and it was very hard to disassemble.There were no reported adverse consequences to the patient.No additional information could be provided.
