The device was returned to olympus for evaluation/inspection.Based on the results of the investigation, the rust was identified.It is likely the result of degradation; however, a definitive root cause could not be established.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
Event or Problem Description
It was observed that during the device evaluation, the lithotripsy transducer exhibited rust on threaded end of the transducer where the ultrasonic probe is attached.There was no patient involvement.
