Event or Problem Description
According to the reporter during a laparoscopic inguinal hernia repair, the single use 30 tacks device misfired.A new 5mm 15 tacks fixator was used to resolve the issue but also misfired.The surgery was delayed a bit while trying to find a device, then a new tackler was opened and used to resolve the issue.There was no patient injury.
