Event or Problem Description
It was reported that after inserting the catheter, health professional flushed it with heparin, but since the solution pooled near the insertion site, health professional concluded that the catheter had ruptured and removed it.Upon inspecting the catheter after removal, health professional confirmed that it was leaking at approximately 34.5 cm.There was no reported patient injury.
