H11: manufacturing review: the device history records have been reviewed and this lot met all release criteria.Investigation summary: the physical device was not returned for evaluation.Four x ray images were provided and reviewed. The x rays were two different mammograms. The provided mammograms demonstrate a clover shaped ultracor twirl post biopsy breast tissue marker in the upper outer left breast. In the first mammogram, the marker maintains its intact clover shape. In the second mammogram, the marker appears fragmented, with the pieces separated from one another, and a new coil shaped marker is visible nearby. The presence of air from the vacuum assisted biopsy device suggests that the second mammogram was obtained immediately following the procedure.Based on the image review, the reported event break can be confirmed. Therefore, the investigation is confirmed for the reported break as the marker broken into pieces were noted in the provided images.A definitive root cause for the alleged break could not be determined based upon the provided information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.B5, g3, h6 (method, result, conclusion).Section a through f: the information provide by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
