Event or Problem Description
It was reported that the patient presented to the emergency department for dizziness and near syncope.A transmission report was reviewed and it was noted that the cardiac resynchronization therapy defibrillator (crt-d) possibly classified an episode of atrial fibrillation (af) with rapid ventricular response as ventricular tachycardia (vt).Additionally, it was noted that the right atrial (ra) lead exhibited undersensing during atrial tachycardia/atrial fibrillation (at/af) stored episodes resulting in termination of at/af detected events in progress and unnecessary pacing at times. It was also reported that the patient experienced phrenic nerve stimulation from the left ventricular (lv) lead. The crt-d and both leads remain in use.No further patient complications have been reported as a result of this event.
