Additional Manufacturer Narrative
|
|
Evaluation of the returned penumbra system red 68 reperfusion catheter (red68) revealed that the catheter was fractured on its distal length. If the red68 is retracted against resistance at angles, damage such as kinks and subsequent fractures may occur.Based on the reported event, the root cause of the resistance could not be determined.Penumbra catheters are inspected during in-process inspection and during quality inspection after manufacturing. The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.
