Event or Problem Description
It was reported that during a comprehensive resection of a right retroperitoneal sarcoma with right nephrectomy and right hemicolectomy, a blue reload used with a reloadable stapler failed to fully seal the anastomosis, resulting in an intraoperative anastomotic leak.Staple formation issues and staple line bleeding after firing were also reported.A new handle and reload was used.
