Investigation summary:
with all the available information, boston scientific concludes that the reported event of not sterile and tear, rip or hole in device packaging was confirmed by media provided. The device has not been received for analysis; therefore, a technical analysis of the complaint device could not be completed.Device history record review:
it was confirmed this device met specification prior to distribution and there were no manufacturing deviations which could have contributed to the reported event.Risk review:
a risk review performed for the versacross device confirmed that the event of not sterile and tear, rip or hole in device packaging are known events defined in the product risk management documentation. This event type has been accounted for during product risk analysis to support acceptable risk benefit for the product.Labeling review:
based on the information provided, there is no evidence that the device was used in a manner inconsistent with the labelled indications/instructions for use (ifu).Investigation conclusion:
boston scientific has assigned an investigation conclusion code of cause traced to transport/storage and supporting evidence that came to this conclusion. The device has not been returned to bsc for analysis, although media provided confirms the allegation. The root cause cannot be determined with certainty based on the information available, however it is likely that the damage occurred during shipping/transport.Sterilization record review was also conducted, and no issues were identified regarding the sterilization process.Due to these factors, it was determined that the sterile barrier breach was not a result of a bsc manufacturing or packaging/handling process but rather the result of shipping damage that occurred outside of bsc control and was easily identifiable prior to use in a patient. The device met all requirements prior to being approved for final distribution/sale and there was no evidence to suggest that the instructions for use (ifu) was not followed.
