Investigation results:
device analysis findings:
the device was returned for analysis.Visual, tactile, and microscopic analysis were performed on the device.A detailed microscopic examination of the balloon material identified a pinhole, located approximately 10 mm proximal to distal markerband.A detailed microscopic examination identified slightly stretching of the inner lumen. No other device issues were identified during returned product analysis.Device history record (dhr) review:
review confirmed that the device met all material, assembly, and performance specifications prior to distribution.Labeling review:
review of the instructions for use (ifu) confirmed there was relevant content and sufficient guidance with respect to the circumstances described within this complaint. No updates are required to the ifu as a result of this event.Risk review:
a risk review was performed and confirmed that the event was defined in the risk documentation. This event type has been accounted for during product risk analysis to support acceptable risk benefit for the product.Investigation conclusion:
this investigation is assigned a most probable investigation conclusion code of cause not established. The information available is insufficient to establish the cause of the reported event. While procedural and anatomical factors cannot be evaluated due to the limited information provided, no definitive cause could be identified.Returned product analysis also identified stretching of the inner lumen, most likely resulting from contact with the guide catheter and guidewire. The most probable cause of this damage is the application of unintended excessive force to the delivery system during device withdrawal.
