Event or Problem Description
It was reported the patient implanted with this neurostimulator experienced shocking sensation below their implant site and buttock area. They reported they felt the shocking sensation when sitting down. The patient denied any falls, procedures, nor received imaging. The patient was assisted with charging their device and no red lights on the remote control were present. Then it was discovered the patient has high impedance. The stimulation was turned off and the physician submitted orders for a revision surgery.Additionally, it was ultimately decided that the patient had surgery to explant their neurostimulator and tined lead with no replacement. Also, it was discovered during explant that the lead was fragmented. The local care team contacted the patient to determine their status, but the patient did not respond.

investigation details:
the device was not returned for analysis; therefore, a technical analysis could not be performed.Good faith effort attempts were made to try and retrieve the device; however, the product was disposed. It was confirmed the device met manufacturing specification prior to distribution and there were no manufacturing deviations which could have contributed to the reported event.Review of the instructions for use (ifu) confirmed there was relevant content and sufficient guidance with respect to the circumstances described within this complaint. No updates are required to the ifu as a result of this event.A risk review was completed and confirmed that the event of high impedance, undesired sensations (non-target stimulation area), and fracture was defined in the product risk documentation. This event type has been accounted for during analysis to support acceptable risk benefit for the product. The media inspection was performed; however, the x-ray image could not confirm any failure or issues related to impedance or stimulation.Based on a thorough review of the reported complaint, boston scientific has assigned an investigation conclusion code of cause not established.Correction: b5.
