Event or Problem Description
It was reported that a patient experienced loss of a dental implant.A fractured implant that had been placed several years earlier by another clinician was removed, and a new implant was placed at the same site.At the 2-month follow-up, implant failure was observed.

Since this event resulted in a serious injury, it is reportable per 21 cfr part 803.The device was not evaluated because the issue is a known inherent risk of the device.We will continue to track and monitor the trend.
