Date of Event 07/02/2026
Type of Reportable Event Serious Injury
Event or Problem Description
It was reported that on (b)(6) 2026, a 29mm navitor vision valve was selected for implant using a flexnav delivery system (ds). The patient did not have a history of right bundle branch block (rbbb), left bundle branch block (lbbb) and 1st/2nd atrioventricular block (av) block. There was no calcification extending beneath the aortic annular plane in the interventricular septum.Pre-implant balloon aortic valvuloplasty (pre-bav) was performed by using 24mm, non-abbott balloon.During pre-bav, the patient was developed with heart block.During the procedure, the valve was able to be implanted at a depth of 4mm on the non-coronary cusp (ncc). The patient remained hemodynamically stable throughout the procedure and there was no clinically significant delay reported.Post-procedure, a temporary pacemaker was placed to stabilize the event.A permanent pacemaker will be implanted. The patient was in a stable condition and subsequently discharged.

Additional Manufacturer Narrative
The device remains implanted in patient. The device will not be returned for evaluation.Investigation is not yet complete.A follow-up report will be submitted with all additional relevant information.

Additional Manufacturer Narrative
An event of heart block was reported intra-procedure.The reported unexpected medical intervention and surgery are results of case-specific circumstances, as temporary pacemaker were performed, subsequently permanent pacemaker was implanted due to heart block. The device history record was reviewed to ensure that each manufacturing and inspection operation was performed, and the product met all specifications.A returned device assessment could not be performed as the device remains implanted and was not returned for analysis.Based on the available information the cause for the reported heart block cannot be conclusively determined. There is no indication of a product quality issue with respect to labeling design or manufacturing of the device.
